Family and Medical Leave Act (FMLA) forms are used by employers to inform employees of their FMLA rights and to certify eligibility for FMLA leave. The deadline on the current model forms from the Department of Labor (DOL) was extended from May 31, 2018 to June 30, 2018. The DOL requested that the model forms be reauthorized without substantive changes for another three years, until 2021.
In the meantime, employers can continue using the current forms.
